Residential duct testing

Has anyone come up with a form,or is there one available, that can be given to the HVAC contractors for the mandatory residential duct testing?

We have been receiving duct leakage test reports for some time now. There does not seem to be a standardized form. Each contractor or testing firm uses a form based on the type of equipment and training they received. As long as the form has the necessary info, we have accepted it.

We have adopted the attached form in Culpeper County for the mechanical contractors self certification. If performed by a certified HERS rater, we will accept their form produced by their software.
